Case Manager RN:

Provide support for the healthcare, providers, office staff, and manager. The Registered Nurse works under the supervision of the Practice Administrator, Nurse Practitioner, and the direction of the Medical Director(s). This position also assists in managing direct patient care and management. The Registered Nurse is also involved with Medical Assistant competencies, proficiencies, and annual evaluations in collaboration with the Nurse Practitioner and Practice Administrator

  • Assist providers in preparing for minor surgeries and procedures as requested
  • Demonstrate knowledge of the principles of growth and development over the life span and possess the ability to differentiate the care that is required based on the patients’ current place on the life growth and development flow
  • Administers nursing care to patients; includes assessment, planning, implementing, and evaluating the patient’s care plan in coordination with the physician.
  • Performs clinical skills competently including:
     
    • Follow up calls for TOC including ED and SJH Discharge patients
    • Medication refills
    • Triage of clinic patients
    • Procedures / patient education as specified by site
    • Triage phone calls and document advice given and disposition of the patient
    • Triage walk-in patients
    • Respond to emergent situations in the clinic
    • Utilized electronic health record
    • Book patient appointments as needed
    • Conduct follow-up phone calls to patients and document in EHR
    • Assist with protocol development
    • Assist with QI activities
    • Participate in clinic team projects
    • Insert IV’s and administer IV medications/fluids per provider order
    • Organize student nurse precepting experiences

 
Experience Required:

  • Graduate of an accredited school of nursing
  • Bachelor’s Degree in Nursing (BSN)
  • Current Colorado RN license or compact license with the ability to obtain Colorado RN license
  • Current BLS certification

Experience Preferred:

  • Minimum of two (2) years of experience as a Registered Nurse in a Family Medicine office practice
  • Experience working with, and sensitivity toward, special populations, including those with limited income, and limited education, and with non-English speaking individuals
  • Bilingual English/Spanish
